This build is based around Fungal Spore – obsidian enchant makes it trigger every time you apply poison, while also keeping you alive with some solid lifesteal. Infused Bracers is using to ramp up the damage. The rest of the setup helps poison trigger more often and stack regen so the poison doesn’t eat you alive. Vital Renewal is a nice bonus skill here – it helps deal with the poison you’re applying to yourself.
